I've never tried ... I've just used Google:
site:lists.xml.org/archives/xml-dev quiz schema

Unfortunately given the topic, the answers appear to be *about*
schema not schemas for such things.

I see there "SPIDER - Structured Platform-Independent Data Entry and
Reporting" and "Triple-s XML Survey Interchange Standard" which might do.
