We really haven't forgotten you, but we're finding a lot more dust under the rug (complications with MS-Word styles dragged through multiple Word versions, and other cruft) as we try to update things...
We have a proposed model under discussion now, so we should be able to give you some closure on the appearance soon, likely by the end of this week.
(We'll still have a lot to do with our MS-Word style sheets to implement the appearance, but that should not have other impact on the DITA-specific work after we provide you with the update.)
I think Scott McGrath has just sent an announcement about one element that's been taking some staff time lately...
