It might be that you are producing XHTML, but parser in browser treat it as HTML, because of wrong mime type or extension.

You are right, I was looking at the DOM inspector in Chrome.
When I look at the source everything is correct.
Which made me check it with a couple of other browsers.
I IE, it is *displayed* correctly; with Chrome, Firefox, and Opera the [1] is displayed without being superscripted. The DOM inspector for those browsers put the [1] outside the 'sup' tag.

Probably yes, but probably not in the stylesheets.
Are you sure that you were looking on the direct result of
transformation and there were no additional step applied to HTML like
HTML-tidy?
Have you been looking at HTML source or have you been using DOM
inspector in your browser?
