Canada 1 and 2

I have corrected the example as suggested in comment 1 from Canada:

In the sample on page 79

</text:change-end text:change-id="c001"/>

now reads

<text:change-end text:change-id="c001"/>

The other comment from canada, a grammtical error on page 94 (section 
5.8) has been corrected already while applying the changes of UK comment 4.
