Yes, support for this requires an extension. Or post-processing.
If you don't have extensions enabled, the mediaobject is turned
into:

   <a xlink:type="simple" xlink:show="embed" xlink:actuate="onLoad"
   href="{$filename}"/>

which you could expand with a <pick your favorite hacking language>
script.
