As has been pointed out, the definition of "reference in DTD" in section
4.4 explicitly excludes the case where it is in an entity value, which
is another row of the table.

The history of this is that the authors of the XML spec wanted a simple
way to constrain PE substitution (outside strings) to whole tokens;
at one point every DTD production had amazingly hairy annotations
indicating where PEs could occur.  The "add a space each side" rule
was intended to be a simple way to provide that constraint.
