A namespace does not require a XSD schema (or a schema in any other
language.) A schema does not require a namespace (you can have more than
one schema for the same namespace, and certainly more than one schema
for no namespace.)

ElementFormDefault only applies to how names are interpreted in an XSD
schema. It does not have any relevance to how namespaces or prefixes are
used in instance documents.
