Index File First Draft

Alright folks, I have been working through the index file trying to get it
into shape.  I have added a table of contents in the beginning.  This TOC
needs to be checked to make sure its accurate.  It really helps us see the
layout of the document, Tim and I will be working on that over the next
couple weeks.  All comments and corrections are welcome.
